Intake Form Questionnaire
First things first! Before Bret can create your 100% customized training program, you will need to fill out a brief intake questionnaire form.
Please click HERE to retrieve your intake questionnaire. It’s important to be as thorough and honest as possible when completing this form. Incomplete information or information that requires clarification will cause a delay in processing your intake questionnaire.
Your Personalized Program PDF will be delivered within 7-days of submitting the completed intake questionnaire form. Bret’s assistant will contact you if there is a delay in creating your program. Please submit this form in a timely manner to avoid any unnecessary processing delays.

Beginning Your New Program and Accessing the Exercise Library
All programs will run on a 4-week cycle. Your first week of every cycle should be considered a “deload” week, which means your effort will be toned down as you are familiarizing yourself with your new program and allowing your body to recover from your previous workout programming. Gradually ramp up the effort during weeks 2-4. Your last week of the program should be the most intense in effort and hopefully you will be setting some new PR’s (personal strength records).
All personalized programs will include access to the Personalized Programming Library. You may access the library HERE. Please note that this page is only accessible if you are logged into your account and if you have an active Personalized Programming membership. You can bookmark this page for easy access.
Before you begin your first workout, please take the time to thoroughly review your program and watch Bret’s video tutorials for the exercises as needed.
As this product is not a 1:1 coaching relationship, discussion, questions, or other general interactions with Bret will be limited. If you require clarification about your program, please email admin@bretcontreras.com and Bret’s assistant will be happy to help. Form checking evaluation and feedback is not available with this product.
Personalized Programming Renewal and Billing
Billing for this monthly product runs on a 30-day cycle, which means subsequent billing for Personalized Programming will occur automatically every 30-days.
If you wish to discontinue this service, please cancel your subscription prior to your next billing date. No refunds will be issued for failure to cancel your subscription. You can manage your account on the Bret Contreras website HERE and manage your membership by clicking on the 'Manage Memberships' link.
A follow up questionnaire form will be emailed to you upon subscription renewal. Please complete and submit this questionnaire at your earliest convenience.
Your next set of workouts will be sent within 7-days of submitting your completed follow up questionnaire. Failure to submit the questionnaire in a timely manner will result in a processing delay and you may experience a gap in your programming.

Your next billing will automatically occur each month. Please cancel your subscription account prior if you no longer wish to receive the Personalized Programming with the Glute Guy product. You will receive an email notifying you of the renewal of your subscription.
